For account verification, use the email, medical record number or phone number on file with your Adventist Health care provider. You will not be able to verify with email address if it is already in use by another patient in the portal.

Self-enrollment is available for Adventist Health patients age 18 or older who do not currently have access to MyAdventistHealth.

Self-enrollment is not currently available to caregivers or guardians. If you manage the health of a patient, talk to the patient's health care provider during the next visit to receive a personal invitation to MyAdventistHealth.
